Abstract: Background: Deep brain stimulation (DBS) surgery is a treatment in the advanced stage of Parkinson's disease (PD). Despite close follow-up, multiple evaluations, and several months of timeconsuming programming sessions, the clinical response may not be optimal in some cases. Imageguided programming (I-GP) could help optimize programming sessions with better clinical outcomes. Objective: To evaluate clinical and quality of life (QoL) outcomes with I-GP in PD patients with DBS of the subthalamic nucleus (STN) and suboptimal clinical improvement and refractory symptoms to conventional clinical programming. Methods: A prospective study in 16 patients with a diagnosis of PD with STN-DBS and remaining adverse effects or symptoms despite a clinical programming adjustment. We simulate potentially effective stimulation based on volume tissue activated (VTA) using commercially available software tools. Clinical outcomes were assessed with motor and quality of life scales. Results: The most frequent suboptimal symptom was gait disorder 33.3% (n=9), residual motor symptoms 25.9%(n=7), and speech 25.9%(n=7). Statistically significant results (p=0.001) were found after I-GP with change in the global Deep Brain Stimulation – Impairment score (41%), The 8- item version of the Parkinson's Disease Questionnaire (47%), EuroQol visual analogue scale (62%), and motor scale (24%). Conclusions: In suboptimal cases of patients with PD and STN-DBS, especially with ongoing and residual motor symptoms, I-GP could be useful to optimize the programming of DBS and improve patients' QoL.

Functional Electrical Stimulation in the Lokomat using Iterative Learning Control

Abstract: Functional Electrical Stimulation (FES) and robotic gait orthoses are rehabilitation technologies that help patients who have lost their normal gait function due to conditions such as spinal cord injury or stroke. FES promotes active muscle contractions that facilitate the rehabilitation process, while the Lokomat (an automated gait orthosis developed by Hocoma in collaboration with Balgrist University Hospital) provides passive limb movements that help to restore and increase mobility. The goal of this project was to combine both technologies in order to improve the beneficial effects that they provide individually. For that purpose, a control algorithm was developed for real-time Linux using an Iterative Learning Control approach in parallel with feedback control. The system was implemented on the ankle and knee joints. The ankle was controlled using a desired angle based on medical literature, while the knee was controlled using a reference force trajectory measured on subjects inside the Lokomat under high-effort conditions. As an alternative to force control on the knee, angle control was implemented for experiments in the lab. The controllers were tested on healthy subjects with and without the inclusion of voluntary movement. Based on these tests, we show the advantages and disadvantages of the control scheme and the physical setup, and give ideas for further improvement. Resumen: La estimulación eléctrica funcional (FES) y las ortesis de marcha robótica son tecnologías de rehabilitación que ayudan a los pacientes que han perdido su función de marcha normal debido a afecciones como una lesión de la médula espinal o un accidente cerebrovascular. FES promueve contracciones musculares activas que facilitan el proceso de rehabilitación, mientras que Lokomat (una órtesis de marcha automatizada desarrollada por Hocoma en colaboración con el Hospital Universitario Balgrist) proporciona movimientos pasivos de las extremidades que ayudan a restaurar y aumentar la movilidad. El objetivo de este proyecto fue combinar ambas tecnologías para mejorar los efectos beneficiosos que proporcionan individualmente. Para ello, se desarrolló un algoritmo de control para Linux en tiempo real utilizando un enfoque de control por aprendizaje iterativo en paralelo con el control de retroalimentación. El sistema se implementó en las articulaciones del tobillo y la rodilla. El tobillo se controló usando un ángulo deseado basado en la literatura médica, mientras que la rodilla se controló usando una trayectoria de fuerza de referencia medida en sujetos dentro del Lokomat bajo condiciones de alto esfuerzo. Como alternativa al control de fuerza en la rodilla, se implementó el control de ángulo para experimentos en el laboratorio. Los controladores se probaron en sujetos sanos con y sin la inclusión de movimiento voluntario. Con base en estas pruebas, mostramos las ventajas y desventajas del esquema de control y la configuración física, y brindamos ideas para mejoras adicionales.

Abstract: Biological and physiological changes in response to stress are crucial to achieving adaption to environmental constraints. Typical stress responses activate the sympathetic nervous system and the hypothalamic-pituitary-adrenal cortical axis, which aim to maintain homeostasis in response to demands and influence various biological responses at neuroendocrine, cellular, and immune levels. In particular, there is compelling evidence showing the devastating effects of high-stress levels on immune responses and brain functionality. In that sense, data suggest that a sustained stress-physiological response predicts decreases in immunity and leads to allostatic load, the wear, and tear of biological systems. Similarly, in cancer research, chronic stress levels have been reported to impair the bodysurveillance systems and be related to cancer incidence, progression, and mortality. Despite the evidence demonstrating the associations between stress and cancer, there are limited data on whether lifetime stress exposure relates to clinical indicators of cancer aggressiveness. Considering the evidence showing the adverse effects of prolonged stress exposure on cancer disease, this thesis aimed to explore the relationship between lifetime stress exposure and the clinical characteristics of tumor-aggressiveness in a cohort of patients affected with triple-negative breast cancer (TNBC). Likewise, as research has shown that coping responses moderate the stress-physiological response, we aimed to investigate the role of coping responses in moderating the association between tumor aggressiveness and lifetime stress exposure. According to the scientific literature, high-stress levels would increase physiological arousal and allostatic load favoring a debilitated immune system. Thus, we hypothesized finding positive associations between lifetime stress exposure and tumor-aggressiveness. In the same line, we suspected that adaptative coping styles would be negatively related to tumoraggressiveness. The participants of this study were 29 women diagnosed with TNBC who were treated in the University Hospital for Gynecology Pius-Hospital (Oldenburg, Germany). Correlation and moderation regression analyses with the use of composite scores were conducted. Overall, we could not demonstrate significant associations between lifetime stress exposure and coping responses on the clinical indicators of tumor aggressiveness in this cohort of patients. Possible explanations for these results are discussed, and suggestions for future research are provided. This study was the first to investigate the effect of lifetime stress exposure and coping response on the aggressive tumor characteristics of TNBC. We provided a deep theoretical framework to understand the interplay between biological and psychological aspects of cancer disease, and we hope that our recommendations may serve as a base for upcoming studies.

The effect of environmental variable selection in the prediction of Seasonal Influenza cases using machine learning

Abstract: Background: Seasonal Influenza is considered to be a cyclic and ordered sequence of values, influenced by external factors that can be predicted and used to detect disease outbreaks and monitoring. In machine learning, the key challenges that limit these analyses are in model explainability and limitations associated with ecological bias. Aim: Determine the best environmental variable selection method to predict Seasonal Influenza in Norway, using an environmental medicine approach combined with machine learning techniques. Methods: This is a quasi-experimental study that compares three approaches (non-variable selection, isolate component, and multipollutant mixture), represented in five methods (univariable, bivariable, multivariable AME, multivariable PCA, multivariable LDA). Per method, the best co-variable combination will be performed, following the internal rules of each method. The best covariable combination is the result of three components: variable selection, validation data set and lag. The first one involves 13 environmental variables (temperature, relative humidity, specific humidity, air pressure, wind speed, precipitation, CO, NO, NO2, O3, PM10, PM2,5 and SO2); second one compares a test dataset compiled from the data from 2019, last year (2018) and a synthetic environmental (avg. 2013-2018) data set in the validation process; and the third one compares a combination of lag from 0 to 12. All the predictions are made using ARIMA algorithm. The evaluation is given in terms of MAE, MSE, RMSE, OR. The training set is from 2 Jun 2013 (week 22/2013) to 28 May 2018 (week 21/2018), and test set is from (week 22/2018) to (week 21/2019) with a window of predictions of 52 weeks. Results: The increment of dimensionality in the environmental variable selection introduce different noise levels and optimize the prediction. Considerations that impact the explainability, usability, ecological bias and performance will be described. Conclusion: The increment of dimensionality in the variable selection has a better impact on performance than using complex algorithms.

In 2007, Mexico City legalized abortion in the first 12 weeks of pregnancy. By doing so, it became the first and only state in Mexico where abortion on demand is legal. In this paper, I use this natural experiment to estimate the effects of abortion legalization on child and maternal health. I perform Difference-In-Differences estimates using information from all births and deaths occurred between 2002 and 2012. My findings suggest that the change in the abortion legislation in Mexico City reduced the Infant Mortality Rate and the Under 5 Mortality Rate by approximately 2.3 and 2.4 deaths per 1,000 live births, respectively. Moreover, for the Maternal Mortality Rate I find that the estimations range from -5.49 to -6 deaths per 100,000 live births. These results are validated by the use of a more accurate control group generated by the Synthetic Control Method. To the best of my knowledge, this if the first paper that attempts to examine the effects of abortion legalization in Mexico City on child health. En el 2007, Ciudad de México legalizó el aborto en las 12 primeras semanas de gestación. Al hacerlo, se convirtió en el primero y único estado del país en donde el aborto voluntario es legal. En este artículo utilizo este experimento natural para estimar los efectos de la legalización del aborto en la salud materno infantil. Realizo estimaciones de Diferencias en Diferencias utilizando información de todos los nacimientos y muertes ocurridos entre 2002 y 2012. Mis hallazgos sugieren que el cambio en la legislación relativa el aborto en la Ciudad de México redujo la tasa de mortalidad Infantil y la tasa de mortalidad en menores de 5 años en aproximadamente 2.3 y 2.4 muertes por cada 1.000 nacidos vivos, respectivamente. Además, para la tasa de mortalidad materna, encuentro que las estimaciones oscilan entre -5,49 y -6 muertes por 100.000 nacidos vivos. Estos resultados se validan mediante el uso de un grupo de control más preciso, utilizando la metodología de control sintético. Según mi conocimiento, este es el primer artículo que intenta examinar los efectos de la legalización del aborto en la Ciudad de México sobre la salud infantil.

Mutations in PCDH19 gene are associated with Early Infantile Epileptic Encephalopathy type 9 (EIEE-9), a very rare disorder characterized by early onset of seizures, intellectual disability and psychiatric comorbidities with an unusual form of X-linked inheritance in which only females and mosaic male patients are affected. Materials and Methods: We report three patients, one male and two females carrying PCDH19 variants (D233G, V589Cfs*8 and N1103K) found by Whole exome sequencing (WES). One is a novel missense mutation (N1103K) while the others had been previously reported in female affected patients. Although, there is the first time that variant V589Cfs*8 is reported in a male patient. We performed a complete phenotype evaluation of our cases and establish the pathogenicity of each variant by in silico and functional analysis accessing many aspects of Protocadherin-19 including subcellular localization and protein expression. Results: Two of the three variants are predicted to be deleterious to protein function. The variant D233G is predicted to affect the calcium binding site and avoids cadherin strands swapping compromising the cell adhesion activity and the structural stability of the protein. While, the variant V589Cfs*8 introduced a premature stop codon which led to the loss of the transmembrane domain of the protein as well as to the cytoplasmic domain, including all predicted nuclear localization signals, suggesting the variant might affect the subcellular localization of Pcdh-19. The last findings were corroborated by western blotting and immunolabelling comparing the protein localization in patient and control fibroblasts. Conclusions: Our findings enhance understanding of molecular functions of Protocadherin-19, as well as highlighting the importance of functional testing of variants that emerge from NGS studies. Resumen: Las mutaciones en el gen PCDH19 están asociadas con la encefalopatía epiléptica infantil temprana tipo 9 (EIEE-9), un trastorno muy raro caracterizado por la aparición temprana de convulsiones, discapacidad intelectual y comorbilidades psiquiátricas con una forma inusual de herencia ligada al cromosoma X en la que solo las mujeres y mosaico pacientes masculinos se ven afectados. Materiales y métodos: Reportamos tres pacientes, un hombre y dos mujeres con variantes en el gen PCDH19 (D233G, V589Cfs*8 y N1103K) encontrados por secuenciación de exoma completo (WES). Una es una nueva mutación sin sentido (N1103K) mientras que las otras habían sido reportadas previamente en pacientes femeninas afectadas. Aunque, es la primera vez que se informa la variante V589Cfs*8 en un paciente masculino. Realizamos una evaluación completa del fenotipo de nuestros casos y establecimos la patogenicidad de cada variante mediante análisis in silico y funcional accediendo a muchos aspectos de la Protocadherin-19, incluida la localización subcelular y la expresión de proteínas. Resultados: se predice que dos de las tres variantes son perjudiciales para la función de la proteína. La variante D233G afecta el sitio de unión al calcio y evita el intercambio de hebras de caderina que comprometen la actividad de adhesión celular y la estabilidad estructural de la proteína. Mientras que la variante V589Cfs*8 introdujo un codón de parada prematuro que condujo a la pérdida del dominio transmembrana de la proteína, así como al dominio citoplasmático, incluidas todas las señales de localización nuclear esperadas, lo que sugiere que la variante podría afectar la localización subcelular de la proteína. Los últimos hallazgos fueron corroborados por Western blot e inmunomarcaje comparando la localización de la proteína en fibroblastos de pacientes y controles. Conclusiones: Nuestros hallazgos mejoran la comprensión de las funciones moleculares de Protocadherin-19, así como resaltan la importancia de las pruebas funcionales de las variantes que surgen de los estudios de secuenciación de siguiente generación (NGS).

This thesis examines the concept of medical futility and its importance for the practice of medicine nowadays. It presents an overview of the development of the concept throughout history and it discusses the main arguments in favor and against a determination of futile treatment in the clinical setting. The notion of futility, in broad terms, describes the situation in which certain medical interventions no longer benefit the patient and therefore, it follows that there must be a derived moral obligation to stop said intervention. Such an idea brings forward numerous problematic pitfalls that are discussed in this thesis, among which are the debate about the limits of medicine, the tension between the notions of patient autonomy and physician authority and the practice of medical rationing in hospitals. This thesis aims to provide a new perspective on the subject in hopes that it will help to keep the ongoing debate running so in the near future, more experts will decide to contribute themselves with their expertise. Situations where a medical intervention ceases to benefit the patient will continue to occur at the bedside; what is more, these ethical conundrums will become more and more problematic as the technological imperative intensifies and the possibilities of biotechnology increase. The leading question that directed the development of the thesis was “when enough it’s enough?” in other words, what are the ethical considerations on the limits of medicine? The paper concludes that the concept of medical futility has a heuristic value, and thus it has helped yield more clarity about relevant matters such as the differentiation between futility and rationing, the fact that more research is needed to test what works and what does not and it has brought to the spotlight the importance of more robust standards of practice that, for example, prevent physicians from practicing defensive medicine. Resumen: Esta tesis examina el concepto de futilidad médica y su importancia para la práctica de la medicina en la actualidad. Presenta una visión general del desarrollo del concepto a lo largo de la historia y discute los principales argumentos a favor y en contra de la determinación de un tratamiento fútil en el ámbito clínico. La noción de futilidad, en términos generales, describe la situación en la que ciertas intervenciones médicas ya no benefician al paciente y, por lo tanto, se deduce que debe existir una obligación moral derivada de detener dicha intervención. Esta idea plantea numerosos asuntos problemáticos que se discuten en esta tesis, entre los que se encuentran el debate sobre los límites de la medicina, la tensión entre las nociones de autonomía del paciente y autoridad del médico, y la práctica del racionamiento médico en los hospitales. Esta tesis pretende aportar una nueva perspectiva sobre el tema con la esperanza de que esta ayude a mantener el debate vigente, de manera que, en un futuro próximo, más expertos decidan aportar su experiencia. Las situaciones en las que una intervención médica deja de beneficiar al paciente seguirán; más aún, estos debates éticos se volverán cada vez más problemáticos a medida que se intensifique el imperativo tecnológico y aumenten las posibilidades de la biotecnología. La pregunta principal que dirigió el desarrollo de la tesis fue ""¿cuándo es momento de parar?"", es decir, ¿cuáles son las consideraciones éticas sobre los límites de la medicina? El documento concluye que el concepto de futilidad médica tiene un valor heurístico y, por lo tanto, ha contribuido a aclarar cuestiones pertinentes como la diferenciación entre futilidad y racionamiento, el hecho de que se necesita más investigación para probar lo que funciona y lo que no, y ha puesto de relieve la importancia de contar con normas de práctica más sólidas que, por ejemplo, impidan a los médicos practicar la medicina defensiva.

Abstract: Latin American countries are experiencing a significant increase in the reports of mistreatment that women experience while using the maternity healthcare services, a phenomenon acknowledged as “Obstetric Violence” (OV). The Latin American region is crucial for the recognition of OV, not only because the term OV was originated there, but also because of its particular response to the matter. The present Scoping Review aimed to identify and review the available evidence about the response of Latin American countries to OV. As a result, it was found, firstly, that the emergence of the concept of OV in Latin America demonstrated to be a transformative tool useful to raise awareness about the mistreatment women face when giving birth. This OV concept also positions this problematic as subset of gendered violence and as a type of structural violence, through which the need to address it systemically is highlighted. Secondly, the used categorization of OV appears to be the broadest and more inclusive typology, emphasizing that OV might stem from both intentional and unintentional actions of healthcare professionals, as well as from conditions within healthcare organizations. Moreover, the identified variables that influence the response to OV in Latin America were synthetized into a theoretical framework, which offers insight into how the problem has been addressed until now and subsequently, provides useful lessons for other countries seeking approaches to combat mistreatment of women during childbirth. Lastly, it was determined that, in order to address OV and allow the progression of public policies to become regionally available, a broadened view encompassing a human rights-based approach is necessary. This research allowed to conclude that OV is a multi-faceted and complex phenomenon whose solution requires a multidimensional and multidisciplinary approach. Furthermore, OV should be understood as more than a simple act of mistreatment, but rather be viewed as a set of socially constructed symbolic, and violent, meanings allowing different stakeholders to reframe the abuse and violence that women undergoing childbirth experience as natural, expected, and accepted, reinforcing specific gender dynamics not merely in maternity care but in society overall. Finally, the framework to tackle OV within the human rights approach developed by the author is expected to be advantageous for the development of further research, programs, and policies to prevent OV in Latin America and beyond.

Abstract: The amygdala is a key hub for the acquisition, storage and extinction of fear memories, which are important in adaptive behaviors. The intercalated cell clusters (ITCs) are groups of inhibitory cells within the amygdaloid complex that have been recently shown to play an important role in fear-related states, especially in the context of fear extinction. Apart from the canonical view that points ITCs as feedforward and feedback inhibitors between the input and output stations towards motor centers, it has been starting to suggest that they could have diverse outputs to different regions beyond the amygdaloid complex. Although some preliminary studies have given some indications of this potential new role of the ITCs, indicating an increase in the complexity of their function in fear behavior and emotional processing, very little is still known. In this thesis, two ITC clusters –lateral and dorsomedial– are the object of the study and two main questions are addressed: (1) Do lateral and dorsomedial ITCs (latITCs and dmITCs) extend long-range projections towards the non-lemniscal (NL) thalamus and the dopaminergic (DA) midbrain? If yes, are their innervation patterns different from each other across these structures? I evaluated the outward latITCs and dmITCs projections to the NL-thalamic and DA-midbrain subnuclei, using anterograde tracing upon Cre-dependent expression of ChR2-EYFP in FoxP2-Cre transgenic mice and less extensively, using retrograde tracing to confirm the results from the anterograde tracing. With these methodological approaches I showed that latITCs and dmITCs do send long-range projections in a differential fashion towards the distinct examined subnuclei. I also showed that these projections are inhibitory (most likely GABAergic) and also, that to assess the innervation pattern, it is necessary to make either automated counting of the fibers, or measure the general innervation in terms of at least two variables–area coverage and fibers’ density– that indirectly provide a more comprehensive innervation index. Resumen: La amígdala es un centro fundamental en la adquisición, almacenamiento y extinción de memorias de miedo, que son importantes para los comportamientos adaptativos. Los grupos de células intercaladas (ITCs) se localizan del complejo de la amígdala, y la regulación inhibitoria sobre ésta, ha demostrado demostrado tener un papel importante en los estados relacionados con el miedo, especialmente en el contexto de la extinción. Aparte de la visión canónica que señala a las ITCs como inhibidores retroalimentadores y proalimentadores entre las estaciones de input y output hacia los centros motores, se ha empezado a sugerir que podrían tener diversas salidas hacia diferentes regiones más allá del complejo de la amígdala. A pesar de que algunos estudios preliminares han dado algunos indicios que soportan este nuevo rol de las ITCs, indicando un aumento de la complejidad de su función en el comportamiento del miedo y el procesamiento emocional, actualmente se conoce muy poco sobre este rol extendido. En esta tesis, dos grupos de células intercaladas diferentes -lateral y dorsomedial- son objeto de estudio y se abordan dos cuestiones principales (1) ¿Los ITCs laterales y dorsomediales (latITCs y dmITCs) extienden proyecciones extendidas hacia núcleos no lemniscales del tálamo (NL) y el cerebro medio dopaminérgico (DA)? En caso de hacerlo, ¿se presenta un patrón inervación diferencial hacia estas estructuras? Para responder esto, he evaluado las proyecciones de los grupos latITCs y dmITCs hacia múltiples subnúcleos del tálamo no lemniscal y del mesencéfalo dopaminérgico, utilizando el trazado viral anterógrado tras la expresión de ChR2-EYFP, que es dependiente del sistema Cre, en ratones transgénicos FoxP2-Cre y, de forma menos extensa, utilizando el trazado retrógrado para confirmar los resultados del trazado anterógrado. Con estos enfoques metodológicos me fue posible demostrar que los grupos de latITCs y dmITCs envían proyecciones extendidas hacia los distintos subnúcleos examinados, de forma diferencial. También demostré que estas proyecciones son inhibitorias (muy probablemente GABAérgicas) y también, que para evaluar el patrón de inervación, es necesario hacer un recuento automatizado de las fibras (por medio de un algoritmo), o medir la inervación general en términos de al menos dos variables -cobertura de área y densidad de fibras- que de forma indirecta, proporcionan un índice de inervación más completo.

Abstract: This research proves that the notion of attunement constitutes an interdisciplinary encounter/space between musical and theatrical training. On this dissertation, I take a look at several notions of attunement to prove It is a physical phenomenon that implies a bodily transformation. Dalcroze's method is the point of departure to define a historical conception of physical training in music and theatre education. However, further developments of his contribution to theatre consider training as a psycho-physical attunement; a method to achieve corporeal plasticity. Next to that, meaningful findings on the study of motion and action in the field of cognitive neuroscience show that the brain works under its independent plasticity, suggesting a more complicated process that attunes two distinct intentionalities in the relation body/mind. This scientific conception of attunement exposes an intricate dialogue between inner and outer bodily dimensions and reveals a relation between individual and collective processes of communication. Neuroscience call to this the mirror mechanism; a system that explains how the process of synapsis occurs as a complex integration that depends on the relation of action and perception of self, others and the environment. Finally, all this knowledge related with instrumental practice will be analyzed and compared with the outcomes of two cases of study; a workshop with members of the Youth Philharmonic Orchestra of Bogotá 2015-2018 and the experience of the performance ‘Blended Affor-dances of Gozo’ at the Royal Central School of Speech and Drama in 2019. Based on this study, I will demonstrate the suitability of conceiving instrumental practice as a collective physical training. A possible emerging method that conceives the development of artistic skills through the practice of physical activity, beyond the childhood period, in later stages of life.
